3. Email form to info@lindahawleyaprn.com

2. Complete items 1 through 21
3. Complete items 22-26 if you have other coverage
4. Email form to Linda - when she returns the form-
5. Be certain to sign in block 27
6. Include your receipt
7. Include the following written on your receipt:
   1. Relationship to insured "Self" or "Mother"
   2. Condition being treated
   3. Type of service rendered i.e. Refill Appointment/Psychiatric Assessment etc.
